Ongoing conditions

Chronic Care

Care for the conditions you live with rather than the ones that pass. You see the same provider each visit, your labs and EKG are done here, and your medications are reviewed as a whole rather than one at a time.

We care for adults 19 and older

What is included

Conditions we manage

The conditions that need steady attention, tracked over time rather than at a single visit.

  • Type 2 diabetes and prediabetes, including A1C monitoring and medication adjustment
  • High blood pressure, followed between visits rather than only at the desk
  • High cholesterol and cardiovascular risk, reviewed with your numbers in front of you
  • Thyroid conditions, including dose changes and the follow-up labs they need
  • Asthma, COPD, and ongoing allergy care
  • Heart disease and arthritis, managed here and coordinated with your specialists
  • Every medication you take reviewed together, including what another provider started
  • Refills handled without a separate appointment once a condition is stable

Why you come back every three months when you feel fine

Most chronic conditions have no symptoms until they have already done damage. High blood pressure, high cholesterol and early diabetes all feel like nothing at all, which is exactly what makes them dangerous. Feeling fine is not evidence that a condition is controlled. It is the normal state of an uncontrolled one.

A visit every three to six months is what makes the difference measurable. Blood work shows whether a medication is doing what it should, whether a dose needs to move, and whether something new has started. Between those visits your own readings at home matter more than anything taken once in an exam room, which is why we ask you to bring them.

The practical version: the appointment is not to confirm that you feel well. It is to catch the thing you cannot feel yet, while it is still easy to change.

How it works

How your care works here

From your first visit to a rhythm that fits your condition.

  1. Your first visit

A full history, a review of every medication you take, and the labs your conditions call for. This visit runs longer than a follow-up, on purpose.

  2. Your plan and your numbers

Targets you can actually check, written down: what you are aiming for and what to do if a reading falls outside it.

  3. Between visits

Bring your home readings. A blood pressure log or a glucose meter says more about the last three months than a single reading taken here.

  4. Your follow-up

Usually every three to six months. We look at your labs and your readings together, adjust what needs adjusting, and set the next date before you leave.

Questions patients ask

  • How often will I need to come in?

Usually every three to six months once a condition is stable, and more often while a medication is being adjusted. If you are well otherwise, that is often the only regular visit you need.

  • I already see someone for this. Can I switch?

Yes. Bring the name and phone number of your current provider and any recent lab results, and we will request your records so nothing starts over.

  • Do you work with my specialist?

Yes. We refer when a condition needs a cardiologist or an endocrinologist and keep managing the rest, so your results and your medication list stay in one place.

  • Can you handle my refills?

Yes. Refills for a stable condition are handled without a separate appointment. If a medication needs monitoring, we will tell you which lab is due and when.

  • Do I have to go somewhere else for blood work?

No. Blood work, urinalysis and EKG are done here during your visit, so the results come back to the provider who ordered them.

  • Do I need a blood pressure monitor at home?

For high blood pressure, yes, and it is one of the most useful things you can buy. Readings taken at home over several days are a better picture than one taken in an exam room, where blood pressure often runs higher.
